Community Health Improvement Partners selects Infotech as their technology partner for the Safety Net Connect project:

By partnering with Community Health Improvement Partners (CHIP) we were able to create a web based referral application for the Safety Net Connect project.  The referral system includes the ability to electronically exchange information between the hospitals and clinics.  Care coordination has improved by assuring that patients leaving the emergency room without a medical home will be able to receive appropriate follow-up care.  Repeat emergency room visits are expected to decrease because the patient now has appropriate follow-up care.  Future emergency room visits are also expected to decrease because the patient will have a medical relationship with a community clinic.
