University of California - San Diego (UCSD) Health System partner with Infotech to create a speciality referral system:

By partnering with University of California - San Diego (UCSD) Health System we were able to create a referral system between UCSD hospitals and local community clinics.  The referral system includes the ability to electronically exchange information between the hospitals and clinics.  The application also allows Primary Care Providers (PCPs) in the clinics to refer patients to hospital specialists.  Several improvements have resulted from the referral system.  Care coordination has improved by assuring that patients visiting the emergency room without a medical home will be able to receive appropriate follow-up care.   PCPs are able to assure that patients receive appropriate specialty care through the specialty referral portion of the system.  Several other benefits are expected due to the use of the application.  Repeat emergency room visits will be expected to decrease because the patient now has appropriate follow-up care.  Future emergency room visits will also be expected to decrease because the patient will have a medical relationship with a community clinic.
